    Department of Anesthesiology and Reanimatology No. 1

    In the early 60s, a new medical science and specialty - anesthesiology and resuscitation (A&R) - began to develop intensively in our country. On the initiative of Professor I. I. Shafer, in 1967 at the Department of General Surgery at the KrasSMA, a course was introduced to teach A&R to students of the 2nd or 3rd year. The anesthesiologist-resuscitator assistant I.P. Nazarov was introduced to the staff of the department. From 1967 to 1974, teaching was carried out on the basis of GB No. 1, from 1975 to 1980 - in the newly opened GBSMP, and from 1980 to 1987 - in the road hospital st. Krasnoyarsk and medical unit No. 7. During these 20 years, about 15 thousand students have been trained on the A&R course. All these years, the assistant taught I.P. Nazarov, who at the same time was in charge of the departments of A&R, was active in medical and scientific work. In 1970 he defended his Ph.D. thesis "State of acid-base balance in children with various types of anesthesia", and in 1984 - doctoral thesis on "Long-term anti-stress therapy with gangliolytics in the preoperative period during and after surgery." In January 1986, he was elected by competition as a professor of the Department of General Surgery, and in July 1997 he was awarded the diploma of the Academician of the MANEB.

    In 1986, at the Department of General Surgery, simultaneously with teaching A&R to students, a 5-month specialization cycle for doctors was introduced. Professor I.P. Nazarov was appointed responsible for this cycle. In 1987, the A&R course was transformed into an independent department of anesthesiology and resuscitation of the FPK and PPS.

    The staff of the department is fully staffed. The head of the department is Doctor of Medical Sciences, Professor, Academician of MANEB I.P. Nazarov with 32 years of teaching experience, 37 years of practical work experience in the specialty. The team includes Associate Professor E.V. Voloshenko and 5 assistants (Ph.D. E.V. Pugonin, Ph.D. D.V. Ostrovsky, E.O. Vasilieva, S.V. Kashcheev, E . V. Ilchenko). The employees are experienced specialists with practical experience from 10 to 25 years, pedagogical - from 5 to 13 years.

    One of the first in Russia, the department began training medical interns in A&R at the bases of medical institutes. There are also classes in anesthesia and intensive care in the cycles of obstetrics and gynecology, traumatology and surgery according to agreed programs.

    22 methodological recommendations were published on topical issues of A&R. On the basis of burn resuscitation, a computer version of the "Expert system for the treatment of burn shock" was created. The staff of the department oversees the training of interns at the bases of the Krasnoyarsk Territory, Khakassia and Tuva.

    The staff of the department is actively carrying out research work begun on the A&R course back in the seventies. The main focus of these studies is the topic "Long-term antistress therapy in surgical patients." Within the framework of the scientific topics of the department over the years, more than 400 articles have been written, 7 monographs have been published and are in print, 5 patents for inventions, 82 rationalization proposals have been approved. Completed 2 doctoral dissertations (I.P. Nazarov, 1983; A.A. Popov, 1996), 14 - candidate.

    In recent years, the staff of the department have paid particular attention to the development of the topic of stress-protective and adaptogenic therapy, extracorporeal methods of detoxification and immunocorrection in patients with severe burns. Many methods of antistress protection and intensive therapy, developed at the department, are successfully used in medical institutions of the city, region and country. The recognition of the practical and theoretical significance of the techniques was the proposals of the organizing committees of many congresses and conferences (in Omsk, Krasnoyarsk, Moscow, Nizhny Novgorod, Japan, etc.) to lecture on the use of stress protectors in surgery and cambustiology. The lectures were delivered by I.P. Nazarov and aroused great interest in Russia and foreign countries.

    It can be stated with confidence that the department successfully performs all sections of the work entrusted to it.

    Department of Anesthesiology and Reanimatology No. 2

    Since 1990, a new stage began in the teaching and development of anesthesiology and resuscitation, when the department of emergency care and intensive care was created. childhood FPK and PPS (since 1996 - Department of Anesthesiology and Reanimatology No. 2 FPK and PPS, Head - Professor A. P. Kolesnichenko) in connection with the needs of practical health care.

    Since the establishment of the department, two main goals have been set in front of it: to improve the training of students of the pediatric faculty and pediatricians, anesthesiologists-resuscitators to provide emergency care and intensive care for children with critical conditions of various origins; to develop and introduce a number of organizational and medical-methodological approaches to reducing child mortality in the region.

    The last years of teaching anesthesiology and resuscitation for the specialty "Pediatrics" among 6th year students are devoted to the study of modern general principles pain relief and etiopathogenetic approaches to intensive care in critically ill children. The learning process is undoubtedly facilitated by the fruitful cooperation of the Department of Anesthesiology and Reanimatology No. 2 of the FPK and PPS with the departments of anesthesiology and intensive care of the City Clinical Hospital No. 20 and the Regional Children's Hospital No. 1. Thanks to this, students have an excellent opportunity to master theoretical and practical material in all sections of pediatric anesthesiology. and resuscitation.

    The department is conducting a large methodical work... On all topics, methodological developments for teachers, classroom and extracurricular work of cadets, test control have been created.

    Since the organization of the department, in parallel with the cadets, students of the 6th year of the pediatric faculty study.

    The staff of the department, under the leadership of A.P. Kolesnichenko, implemented a new conceptual position on end-to-end training of pediatricians in the medicine of critical conditions of childhood in conjunction with other departments - pediatrics, pediatric surgery, anesthesiology and resuscitation № 1 of the FPK and PPS.

    In 1990-1993 in the region, thanks to the efforts of the department staff and practical health care, a long-term program to reduce infant mortality was developed and implemented.

    On the basis of the regional hospital, for the first time in Krasnoyarsk, a regional center for pediatric anesthesia, resuscitation and intensive care was opened, in the creation of which a doctor of the highest category I.N.Popandopulo rendered great assistance. From the day of opening, this medical unit has become the main base of the department for conducting the educational and medical process.

    Then, on the basis of the center, a resuscitation and consulting center (RCC) was deployed for older children, and subsequently for newborns.

    For the regions of the Territory and Krasnoyarsk, a threat-metric advisory-automated system (UKAS), developed by St. Petersburg and Yekaterinburg specialists, was adapted and implemented.

    At the same time, the computer programs DINAR-1, DINAR-2.11 were introduced, which made it possible to place all children in the region with threatening conditions on dynamic intensive observation and to provide preliminary syndromic diagnosis of critical conditions.

    The department prepared and defended 1 Ph.D. thesis on the topic "The choice of optimal parameters and modes of mechanical ventilation in children, depending on the type of ARV".

    The main directions of the scientific and methodological activities of the department:

    • respiratory support problems in critically ill patients;
    • problems of endotoxicosis correction in critically ill patients;
    • shifts in hemostasis in critically ill patients;
    • the choice of methods of pain relief in the obstetric clinic;
    • improving the organization of the structure of the anesthesiology and intensive care service in order to reduce child and maternal mortality.

    In 1979, a new course of surgical diseases was created at KrasSMA to improve the qualifications of doctors, which gradually grew into the department of advanced training for doctors.

    The organizer and head of this department was Doctor of Medical Sciences, Professor OE Nifant'ev. The teaching was carried out on the basis of the road hospital of the Krasnoyarsk station and the city hospital of emergency medical care. Basically, the department was engaged in the general improvement of doctors in surgery through GO on a 4-month cycle.

    In 1986, the department moved to the base of the city clinical hospital No. 20. The staff of the department is changing, a different profile of the clinical base of the hospital No. 20 with many specialized departments allows you to change the topic of teaching. New cycles appear: "Proctology", "Selected issues of surgery for internship leaders", "diagnostics and emergency care in acute diseases and injuries of the abdominal cavity and urinary tract." A new cycle of FPC is introduced for teachers of medical schools, which existed until 1992.

    During this period, the department is engaged in a large research work on the problems of peritonitis. A new method of sanitation of the abdominal cavity, its drainage has been developed, clinical and laboratory criteria for assessing intraoperative measures and indications for programmed relaparotomy have been determined. A fundamentally new device for the express preparation of sanitation solutions "Geyser", which has found wide application in emergency surgery, has been developed and put into serial production.

    Under the supervision of O.E. Nifant'ev, 1 doctoral and 9 candidate dissertations were completed and defended.

    Since 1993, the department has been headed by Doctor of Medical Sciences, Professor A. M. Sukhorukov.

    The clinical base of the city hospital No. 20 has become more specialized: a department of vascular surgery has appeared, the department of proctology has grown into the regional center of coloproctology, and the endoscopic service has grown significantly. The department received another clinical base - the surgical department in the medical unit No. 96. The staff of the department has changed and replenished.

    All this made it possible to significantly change the thematic plan of classes with cadets, taking into account the requests and needs of the regional healthcare. New cycles have appeared: "Emergency surgery of the thoracic and abdominal cavities", "Surgery of the liver, biliary tract and pancreas", "Selected issues of angiology", "Topical issues of endoscopy in the diagnosis and treatment of diseases of the abdominal and thoracic cavities." A lot of work has been done to create programs for these cycles and teaching aids.

    Since 1998, new 3-month certification cycles in surgery and endoscopy have been running.

    Since 1999, the department has additionally received an ambulance course, which is headed by Doctor of Medical Sciences Associate Professor A.A. Popov.

    The department is a base where they are trained annually in an internship in surgery.

    Emergency conditions and purulent-inflammatory diseases of the chest and abdominal cavity organs, angiology and coloproctology remain the subject of scientific research at the department. The department carries out 1 doctoral and 3 master's theses.

    One of the oldest employees of the department is Associate Professor N.I.Davydova. The team also includes associate professor V.A.Fokin, assistants, candidates of medical sciences A.E. Popov, V. Yu. Tolstikhin, V.O. Timoshenko, senior laboratory assistant, candidate of medical sciences E.A.Popova.

    The staff of the department actively cooperate with the regional health authorities and are full of creative ideas and plans.

    Department of Clinical Laboratory Diagnostics

    In 1983, the department of specialization and improvement of laboratory doctors was created at KrasSMI, which in 1985 was renamed the department of clinical laboratory diagnostics, which was evidence of the recognition of clinical laboratory diagnostics as an independent medical discipline.

    The main base of the department all these years is the clinical diagnostic laboratory of the regional clinical hospital No. 1. Part of the classes is carried out on the basis of the leading clinical diagnostic laboratories of the city (regional children's hospital, AIDS center, medical genetic center, city hospital No. 4, city TB dispensary and etc.).

    From the day of opening to the present time, the department is headed by Doctor of Medical Sciences, Professor Yu. O. Toroshchin. The department employs 2 professors, 2 associate professors, 2 assistant candidates of medical sciences.

    During its work, the department has trained a whole group of highly qualified specialists who work both at the department and in medical institutions of the city: they defended their doctoral dissertations and became professors Yu.O. Toroshchin and T.A.Kozhevnikova, received the title of associate professor T.P. Tsygankov, T. I. Udovitsina, V. T. Murnikov, I. A. Olkhovsky, defended his Ph.D. thesis I. A. Zaliznyak. The large clinical diagnostic laboratories of the city were headed by former employees of the department D.A. Grishchenko - chief specialist in laboratory diagnostics region, V. M. Alferov, I. A. Olkhovsky - head of the clinical diagnostic laboratory of the regional AIDS center.

    In just 15 years, the department has conducted 88 cycles (of which over the past 5 years - 42) and trained 13,700 cadets (over the past 5 years - 699 people) from Krasnoyarsk, Krasnoyarsk Territory, other regions and territories of most territories of the former USSR and Russia ( Ufa, Murmansk, Tyumen, Perm, Khabarovsk, Vladivostok, Sakhalin, Kamchatka, Buryatia, Altai Territory, Kemerovo, etc.).

    The qualitative composition of the cadets is heterogeneous: in our specialty there are doctors of the medical and pediatric faculties, doctors of the biomedical faculty, biologists who graduated from the university, teachers of biology, chemistry, physics, sanitary doctors, pharmacists, physicists, chemists, biophysicists and even technical workers.

    In the course of the educational process, computer control and training programs, familiarity with modern laboratory technology are used.

    The department was the initiator of the organization in the Krasnoyarsk Territory of the regional regional association of laboratory diagnostics workers, which is part of the Russian Association.

    The department conducts scientific research in the field of hemostasis, clinical immunology, immunodiagnostics, deals with the problems of neuroimmunology, longevity and environmental immunogenetics. For many years, active scientific research on the problem of atherosclerosis has been carried out jointly with the Research Institute of Medical Problems of the North of the Siberian Branch of the Russian Academy of Medical Sciences. Currently, a new phenomenon in biology and medicine is being studied related to the structuring of biological fluids. The work is carried out jointly with the Institute of Gerontology of the Russian Academy of Medical Sciences, with the KSU; developments in the field of new laser technologies in medicine are underway. Within the framework of the integral program of the country's universities, the department actively participates in the training of medical physicists together with KSU. The most interesting scientific works at the department are related to the problems of psychogenetics. This scientific direction is new, still developing, but interesting results have already been obtained. Scientific research on psychogenetics is carried out jointly with the International Scandinavian University (Sweden, Stockholm), the Department of Medical Genetics of the International Institute of Friendship of Peoples, the Russian Medical Genetic Center of the Russian Academy of Medical Sciences (Moscow).

    The medical work of the department is based mainly in the hospitals of the regional clinical hospital, where the teaching staff consults patients, conducts consultative work among doctors.

    In terms of further development the department is supposed to conduct cycles on management and marketing for organizers of health care of the region of all levels for hospitals and laboratories of all forms of ownership. The department plans to conduct cycles for general practitioners on clinical laboratory diagnostics, narrow-profile cycles on hematology, clinical biochemistry, laboratory diagnostics of emergency conditions. In the light of the concept of integration of departments - in medical and genetic research.

    Department of Radiation Diagnostics

    The department was created in September 1983 on the basis of the course of radiology. Professor L. S. Grakova was appointed as the head of the department, and E. K. Ivanov A. Z. Khromushkin, Z. M. Tyazhelnikova were appointed as the head of the department. Initially, the lecture hall and study rooms were located in the lobby of the first floor of the first building of the regional clinical hospital No. 1.

    In September 1990, the department was provided with a separate block in the polyclinic of the regional clinical hospital No. 1 with a total area of 142.2 square meters. m.

    The need to create a department was dictated by the need for practical health care in the training of specialists in radiation diagnostics.

    Currently, the hospitals have organized departments of radiation diagnostics, which include radiology, ultrasound examinations, computed and magnetic resonance imaging.

    There are 370 offices with a staff of 452 radiologists in the region. Since 1994, ultrasound diagnostics has been actively introduced into radiation diagnostics. There are 173 ultrasound rooms with a staff of 270 specialists in the region.

    In connection with the need for practical health care at the department, training is carried out in the following cycles: specialization in radiology and ultrasound, thematic improvement cycles in pediatric radiology, fluorography, urology, diseases of the osteoarticular system, abdominal organs, etc. cycles. 115 doctors are trained annually.

    Over 15 years, 1170 radiologists have been trained. The department annually graduates 4-5 interns and clinical residents.

    The educational process is combined with the certification of doctors for the qualification category.

    The department performs not only stationary educational process... Taking into account the personnel needs of the regions of the region, field trips are organized. Certification cycles were carried out in Khakassia, Norilsk, Sharypov, Zheleznogorsk.

    Over time, the scientific potential of the department staff was gaining momentum. By the time the department was created in 1983, only the head of the department, L. S. Grakova, was a doctor of medical sciences. By 1999, all teachers have academic degrees.

    The first candidate's dissertation was defended in 1987 by I.G. Grakov and doctoral dissertation - E.V. Galkin. In 1999, S. I. Bestovskaya became a doctor of medical sciences.

    Over the past 10 years, 10 Ph.D. theses have been defended. Two employees of the department - associate professor E. N. Batukhtin, assistant N. V. Gunicheva are working on doctoral dissertations.

    Scientists of the department published 170 printed works, received 8 certificates for inventions, published 3 monographs and 1 teaching aid in the form of an atlas on ultrasound diagnostics for cadets, 30 rationalization proposals were introduced.

    The teachers of the department are well-known and lead an active the international cooperation with colleagues from Japan, USA, China. Joint conferences, exchanges of scientific developments are held.

    Department of Neurosurgery and Neuropathology

    The Department of Neurosurgery and Neuropathology for Advanced Training of Doctors was organized in 1980. The creation of the department was caused by the need to improve neurosurgeons. In addition, there was an urgent need to improve the knowledge of surgeons, traumatologists, ambulance doctors, primary care physicians in urgent neurosurgery and neuropathology, as well as to familiarize senior students with the basics of neurosurgery. However, the formation of the neurosurgical service, which later became the base of the department, began long before its opening.

    In 1969, at the Department of Hospital Surgery, an associate professor's course in neurosurgery was created for classes with 5th and 6th year students.

    NS Draluk was elected associate professor. In the same years, classes were conducted with neuropathologists at the Department of Nervous Diseases (Head of the Department, Professor V.A.Rudnev, Associate Professor A.B. Grinstein). Subsequently, the department was formed from the merger of these two cycles.

    Since the day the department was founded, its bases have been the regional clinical hospital No. 1 and the emergency hospital with a total of 240 beds. The neurosurgical operating room of the KKB No. 1 is equipped with modern equipment that allows performing microsurgical operations for tumors and aneurysms of the brain. There is all the necessary equipment for conducting examinations at a modern level (MRI, CT, angiography, etc.).

    From 1991 to the present, the department is headed by a full member of the New York Academy of Sciences, Doctor of Medical Sciences, Professor M.G. Draluk. Teaching is carried out by 3 doctors of medical sciences, 2 associate professors, 3 candidates of sciences.

    The staff of the department carries out a large amount of scientific research in various areas of neurosurgery and neuropathology: monographs have been published, chapters in various manuals, methodological manuals for practitioners have been written, more than 300 works have been published in various collections and journals, including foreign ones. A conference of neurosurgeons of the Russian Federation, a plenum of the board of the society of neurosurgeons, a Russian-Japanese conference of neurosurgeons were held. For the first time in our country, a long-term intracarotid infusion has been developed and introduced for the treatment of severe inflammatory and vascular diseases of the brain (Professor NS Draluk, 1966). The study of the microcirculatory bed of the brain stem in injuries, vascular diseases and brain tumors has been developed in detail and continues to be improved (Professor M.G.Draluk, 1980). The results of the research carried out were reported at republican and international conferences. He was also the first to study and apply transoperative cytological diagnostics of brain tumors (1979) Together with the Department of Neuropathology, a functional assessment of the regenerative reserves of the brain with its organic lesions is carried out (Doctor of Medical Sciences A.A. Narodov, 1997). A long-term intra-aortic infusion has been developed and implemented for severe concomitant craniocerebral injuries (Professor M. G. Draluk, Ph.D. I. P. Grinev, 1997). Research is being carried out to study the clinical picture and diagnostics of vascular diseases of the brain, diseases of the peripheral nervous system (associate professor A.B. Grinstein).

    Close relationship with practical health care has made it possible to create a powerful neurosurgical service in the region, to increase the knowledge of primary care physicians in the areas of emergency neurosurgery and neuropathology. The Department and Clinic of Neurosurgery continue to develop and improve in full cooperation with the leading institutions of the country, with practical health care, continue to strengthen international ties.

    Department of Dentistry

    The Department of Dentistry FPK and PPS was established on March 13, 1989 on the basis of the 1st self-supporting polyclinic in Krasnoyarsk. The first head of the department was associate professor L.V. Kostromina, then from 1993 to 1998 the department was headed by associate professor V.F. Ostanin, and at present - Associate Professor Yu. V. Chizhov. Since 1995 the staff of the department has remained unchanged: associate professors V.F. Ostanin, T.K. Voloshin, Ph.D. O. R. Sokolova, Ph.D. O. N. Tumshevits.

    Over the years, 812 dentists have undergone thematic improvement, and 120 certificates have been issued.

    The scientific work of the department staff is multifaceted, including the study of the pathology of the oral mucosa and periodontal disease, the prevention of dental diseases in adults and children. In surgical dentistry, speech-improving operations have been developed and introduced in case of insufficiency of the palatopharyngeal valve of various etiologies, devices for the diagnosis and training of the organs of speech articulation.

    19 applications for inventions were filed, 9 patents and 4 positive decisions were received; 11 rationalization proposals were submitted, 80 articles were published, 5 of them - at the international level, 11 methodological recommendations were published.

    Therapeutic work is carried out on the basis of the dental clinic, the departments of maxillofacial surgery of the emergency hospital and the 5th children's surgical hospital. More than 700 patients are consulted annually and 70–75 operations are performed on patients with complex pathologies.

    Associate Professor V. F. Ostanin created a children's department of maxillofacial surgery with a center for the treatment and rehabilitation of children with congenital malformations of the face, where from 500 to 600 children of various ages are periodically treated.

    The staff of the department pay much attention to practical doctors in terms of attracting them to scientific work. We jointly received 3 patents of the Russian Federation for inventions, introduced 13 rationalization proposals, published 21 articles.

    Department of Therapy

    The intensive development of medical science by the end of the 70s urgently required the widespread introduction of scientific developments into the activities of practical health care. Therefore, in 1970, on the basis of the Department of Faculty Therapy of KrasSMA, headed by Professor, Doctor of Medical Sciences. FF Kostyuk, a world-renowned scientist, organized a course of specialization and advanced training for therapists of the region. The aim of the course was mainly the primary specialization of therapists. The course was supervised by a young talented assistant of the department, Ph.D. N. N. Nikolaeva, and classes on the cycle "Therapy" were conducted by such highly qualified specialists as Associate Professor KT Kosolapova, Ph.D. V. V. Kharlamova, Ph.D. I. M. Gankin, Ph.D. V. A. Shulman. Lectures on the main sections of therapy (gastroenterology, pulmonology, cardiology) were delivered by Professor F.F.Kostyuk.

    Deep knowledge of the world medical literature and extensive clinical experience allowed F.F. A. Shulman, S. V. Klemenkov), and in the practice of City Clinical Hospital No. 20, on the basis of which the department was located, for the first time in the region to introduce methods of diagnosis, treatment and resuscitation of patients with acute myocardial infarction, rhythm and cardiac conduction disorders.

    In 1989, there was a reorganization of the Department of Internal Diseases No. 1 and the Department of Therapy FPK and PPS was organized. To fulfill the tasks of postgraduate training, the staff of the department must combine the qualities of an experienced practical doctor, who owns modern methods of examination, case management, and a highly qualified teacher. Therefore, the team was headed by Professor F.F. Kostyuk, and it included Associate Professor N. N. Nikolaeva, Ph.D. V. A. Shestovitsky, Ph.D. E. G. Grishchenko, assistants N. V. Topolskaya, T. F. Zinovieva, I. V. Lipnyagova.

    In 1994, the team was headed by a well-known specialist and scientist, Doctor of Medical Sciences, Professor Yu. I. Grinshtein. With his arrival, the department received further development in educational, methodological, scientific and medical activities. The staff of the department was replenished with new talented specialists, among them assistant V. V. Shabalin, Ph.D. V. V. Kusaev, Ph.D. M. I. Gankin, Ph.D. S. V. Ivliev. Such areas of internal medicine as cardiology, rheumatology, nephrology, pulmonology have been intensively developed.

    Since 1998, Professor Yu. I. Grinshtein became the head of the center of intensive cardiology created in the region on the basis of KKB No. 1, which also significantly expanded the possibilities of postgraduate training of physicians and cardiologists of the region. Under the leadership of the head of the department, a large educational, methodological and research work is carried out.

    The main scientific direction of the department is the development, testing and implementation of new methods of diagnosis and treatment in the clinic of internal diseases. Research work is carried out in different directions, namely in cardiology, gastroenterology, pulmonology and rheumatology.

    Currently, 7 dissertations are being carried out under the guidance of Professor Yu. I. Grinshtein. They are devoted to topical issues of cardiology and internal medicine.

    The role of the permanent head teacher, Professor N. N. Nikolaeva, who has a rich 35-year experience as a doctor and teacher, plays a great role in the life of the department. NN Nikolaeva is a leading gastroenterologist of the city and region, her contribution to applied gastroenterology is very significant. Candidate and doctoral dissertations are carried out under her supervision. Professor N. N. Nikolaeva is the scientific director of the city gastroenterological center, the author of many guidelines for the pathology of the gastrointestinal tract.

    At active participation Department of therapy of FPK and PPS and, in particular, Professor Yu. I. Grinshtein, on the basis of City Clinical Hospital No. 20, a regional rheumatological center was created and is functioning. With the participation of associate professor of the department V.A. Shestovitsky, the city pulmonological service is being developed.

    The staff of the department annually publishes more than 40 abstracts and articles in the local, central and foreign press. Only in recent years the following monographs have been published: Professor FF Kostyuk - "Myocardial infarction", "Angina pectoris", "Memories of a doctor and teacher"; Professor Yu. I. Grinstein - "Systemic vasculitis", "Diagnostics and treatment of glomerulonephritis"; Professor N. N. Nikolaeva - "Therapeutic factors of the resorts of the Krasnoyarsk Territory and the Republic of Khakassia"; Professor FF Kostyuk and Associate Professor VA Shestovitsky - "Bronchial asthma".

    The staff of the department has more than 60 guidelines for doctors, 70 rationalization proposals, 5 patents for inventions. On the initiative of the head of the department, Professor Yu. I. Grinshtein, large Siberian conferences on pharmacotherapy in clinical practice began to be held annually in Krasnoyarsk. They bring together the country's leading experts and are an invaluable school of postgraduate education.

    The educational process at the Department of Therapy FPK and ATS is constantly being improved. During recent years the annual number of doctors studying in 7 thematic cycles has increased to 280-300 people. Since 1996, the department has been conducting certification cycles in the following specialties: therapy, cardiology, gastroenterology, pulmonology; since 1998 field trips to the cities of the region have been organized. The staff of the department is full of creative ideas and will strive for their implementation.

    The strategic objective is to meet the needs of practical health care in the training of qualified personnel on the basis of the following requirements for medical education: 1) compliance with the needs of health care in terms of quantitative and qualitative indicators of training specialists; 2) efficiency; 3) continuity; 4) competitiveness based on quality standards; 5) continuity.

    According to the existing legislative framework, the main stages of postgraduate (postgraduate) education are internship, clinical residency, advanced training and professional retraining, training of specialists.

    Internship, representing the actual medical internship, is a mandatory stage for graduates of medical universities who want to obtain the right to independent medical practice.

    In general, over the past 5 years, the number of specialists who graduated from internships decreased by 3.4%, mainly due to a decrease in the number of interns trained at the bases of medical institutions of the Krasnoyarsk Territory (Table 6).

    During the same period of time, the number of specialists who graduated from clinical internship increased by 25.7%.

    46 specialists (45.0%) have contracts for subsequent employment in primary health care.

    In 2006, in order to improve the quality of training of clinical residents, the system of integration of departments was used in training specialists in fundamental and related disciplines, as well as in the section "public health and health care"; a questionnaire was systematically conducted on the effectiveness of teaching sections and disciplines.

    To solve the organizational problems identified in the analysis of primary specialization in 2005, in 2006: 1) a system of promoting the employment of "young" specialists was developed; 2) control over the quality of contracts for the employment of specialists was carried out; 3) the formation of a system of primary specialization, focused on primary health care, began.

    ^ Professional development (professional retraining) of specialists

    Training and retraining of specialists with medical education at KrasSMA is carried out annually within the framework of the educational and production plan approved by the Ministry of Health and social development.

    Over the past 5 years, the plan for training specialists in the courses of advanced training has been increased by an average of 19.6% (Table 9). At the same time, until 2005, the plan was overfulfilled, reaching 7.9% (2004). In 2006, according to preliminary data, the plan in terms of training specialists in absolute terms was fulfilled by 93.0%.

    ^ Assistance in the employment of "young" specialists

    The negative socio-economic processes that took place in the 90s of the last century undoubtedly affected the state of the human resources in the healthcare sector. Outflow of specialists from state and municipal institutions health care, primarily the territories of the Krasnoyarsk Territory, occurred for the following main reasons:

    1. cancellation of the state distribution of graduates;

    2. non-payment and low level of wages with a sufficiently high responsibility (psychological, legal);

    3. low level social and living conditions in the territories of the region;

    4. division of powers in terms of responsibility for the state of the health care system between state and municipal executive authorities;

    5. the emergence of a private health sector;

    6. reduction of the state (federal) order for the training of specialists with higher medical education;

    7. “Artificial” retention of vacant rates for internal part-time jobs and the reluctance of heads of medical institutions to hire young specialists.
    Thus, by the beginning of 2006, the shortage of doctors of all specialties in the region, as already indicated, amounted to almost 4,000 people with a high percentage of part-time jobs (1.9). Against this background, there remains a low percentage of staffing with specialists providing specialized types of medical services (psychiatrist - 63.3%, phthisiatrician - 58.0%, pathologist - 37.0%, etc.)

    In the process of implementing the priority national project in the field of health care, it was possible to improve the staffing of medical personnel in primary health care (district therapists - 76.2%, district pediatricians - 91.9%).

    In 2005, 474 people graduated from the medical, pediatric and dental faculties on a budgetary and extrabudgetary basis, including 133 off-budget (28.1%).

    ^ For the passage of the primary specialization, contracts have been concluded:

    1. with medical institutions located on the territory of the Krasnoyarsk Territory - 301 (63.5%) graduates;

    2. with medical institutions of the Republic of Khakassia, the Republic of Tuva, the Evenk and Taimyr Autonomous Districts - 92 (19.4%) graduates;

    3. were trained on a commercial basis - 35 (74%) graduates.
    According to the results of the analysis of employment, by conducting written requests to medical institutions of the Krasnoyarsk Territory, specialists who have completed primary specialization, it was found that employed 200 doctors (66.4%), including:

    1. 119 specialists in municipal healthcare institutions of the region;

    2. 51 - in the regional government agencies health care;

    3. 30 - in departmental health care institutions.
    Continuing training (clinical residency) - 45 specialists.

    56 (18.6%) specialists have not been employed, 15 of them are on maternity leave. That is, 41 people (13.6%) find their application in non-state and municipal health care systems.

    37 young specialists were employed in the primary health care unit, including after the activities carried out in the framework of the implementation of the priority national project in the field of health care.

    Nevertheless, in general, I would like to emphasize that 81,4% Doctors graduated in 2005 should be considered employed.

    ^ During the assessment of the results of employment, the following problems were identified that need to be addressed:

    1) the difficulties of not issuing documents on the completion of an internship, residency in case of successful passing of exams, related to the existing legislation;

    2) imperfection of legislation in terms of fulfilling obligations under concluded contracts for a three-year term of work;

    3) the presence of fictitious internship contracts;

    4) lack of measures on the part of the heads of health care institutions to return "young" specialists, in accordance with the concluded contracts.

    So, according to the contracts concluded with the City Clinical Hospital No. 20 of Krasnoyarsk - 10, GP No. 14 of Krasnoyarsk - 6, Kuraginskaya Central District Hospital - 4, Kansk Central City Hospital - 4, etc. to the return of young specialists, in accordance with the signed contracts.

    Graduated in 2006 targeted training55 graduates KrasSMA , including 26 - Faculty of General Medicine, 20 - Faculty of Pediatrics, 9 - Faculty of Dentistry.

    52 (94.5%) people entered into agreements on the passage of primary postgraduate training with the municipal health care institutions of the territories from where they were sent, including:

    • 24 (92.3%) graduates of the medical faculty;

    • 19 (95.0%) graduates of the pediatric faculty;

    • 9 (100%) graduates - specializing in dentistry.
    Until now, 3 graduates of KrasSMA (2 people from the Faculty of Medicine, 1 person from the Faculty of Pediatrics) have not signed an agreement on the passage of primary postgraduate training, which is 5.5% of the total number of graduates who graduated from universities in the targeted enrollment.

    It should be noted that the formation in KrasSMA of a system of assistance in the employment of specialists started from January this year... This work is entrusted to the department of assistance in the employment of "young" specialists of the Institute of Postgraduate Education KrasSMA.

    Over the past period of time, the following activities have been carried out.

    In March 2006, together with the health and drug supply agency of the Krasnoyarsk Territory administration, the distribution of 2006 graduates was carried out, according to the results of which all, except for 3), studying at KrasSMA for targeted contract training, had contracts with the relevant medical institutions for subsequent employment.

    In the 2005-2006 academic year, special attention was paid to working with clinical interns and clinical interns.

    Specialists of KrasSMA held a number of seminars, round tables with interns and clinical residents, first of all, students in the specialties "therapy", "pediatrics", "general medical practice (family medicine)", which discussed the implementation of a priority national project in the field health care, including improving the work and strengthening of primary health care.

    To assess the employment of specialists who graduated from clinical residency and internship in 2006, GOU VPO KrasSMA of Roszdrav sent letters to the heads of medical institutions in August 2006 with a proposal to provide information on the actual employment of doctors.

    In September-October of this year, meetings were held with 6th-year students of the Faculty of Medicine and Pediatrics, 5th-year students of the Faculty of Dentistry on employment issues, orientation of future graduates to primary health care.

    Since March 2006, work has begun on placing information about vacancies in healthcare institutions of the region on the KrasSMA website. There are currently 238 vacancies available. To improve this work, at the end of October, the heads of the regional health authorities and institutions were sent letters with a proposal to provide information on the available vacancies. However, it is required to develop a system of constant monitoring of the closing and opening of vacancies.

    To assess the quality of training of specialists, at the beginning of November this year, a questionnaire "Quality of training a specialist at KrasSMA" was sent to the health care institutions of the region.

    To organize events to familiarize senior students with medical institutions of the territory of the region, the heads of these institutions were sent letters with proposals: on the provision of electronic resources of institutions for posting links on the KrasSMA website and placement on their websites of advertising information intended for students (characteristics of institutions, types, volumes and profiles of medical care, social conditions, etc.).

    During this school year a number of trips with graduates in the territory of the region are planned for the presentation of medical institutions, working conditions and social conditions. Already in December, a trip to Lesosibirsk was made. But there is a problem for carrying out such work - the financial data of the measures (transportation costs, etc.).

    Also, the estimate of KrasSMA does not include the costs of sending graduates to industrial practice in the territory of the region.

    At the beginning of 2007 it is planned individual work to promote the employment of specialists in problem areas and healthcare institutionswith an indicator of staffing of regular posts of district general practitioners:

    below 70%: Bolshe-Uluiskaya CRH, Achinsk CRH, Nazarovskaya CRH, Pirovskaya CRH, Sayansk CRH, Divnogorsk Central City Hospital, Ilanskaya CRH, GP No. 7 of Krasnoyarsk, GP No. 2 of Lesosibirsk - (9 institutions),

    below 60%: Kuraginskaya CRH, Uzhurskaya CRH, Sharypovskaya CRH, GP No. 2 in Achinsk, GP in Minusinsk, GP No. 1 in Krasnoyarsk - (6 institutions),

    below 50%: Kazachinskaya Central District Hospital, Kansk Central City Hospital, Motyginskaya Central District Hospital, Strelkovskaya City Hospital (Lesosibirsk), GP No. 1 of Achinsk, City Hospital of Achinsk, district hospitals of Nazarovsky district - (7 institutions),

    with an indicator of staffing of regular posts of district pediatricians

    below 70%: Achinskaya CRH, Taseevskaya CRH, district hospitals and outpatient clinics of Nazarovsky and Sharypovsky districts (4 municipalities),

    below 50%: Pirovskaya Central District Hospital. - (1 institution).

    In February 2006, it is planned to conduct a survey of students to assess their opinions, what factors (reasons) they are guided by in their desire or unwillingness to work in healthcare institutions of the territory of the region.

    However, in the context of imperfect legislation regarding the implementation of contracts concluded, there is a risk that the effectiveness of the above measures will be no more than 10-15%.

    For a fairly prompt solution to the problems of employment of specialists with higher medical education, primarily with the primary health care, it is advisable to go in two directions:

    1. Stop issuing documents to doctors who have completed primary specialization, starting in 2007, using the following technology: 1) issue to employers (according to concluded contracts) certified copies of documents for hiring specialists; 2) store originals in KrasSMA or in the health and drug supply agency of the regional administration; 3) upon completion of the specialist's fulfillment of obligations to fulfill the contract (after three years), issue original documents.

    2. To form a system of educational loans for applicants, students to finance training costs with the condition of repayment of the loan in the form of a three-year term of work in a medical institution in accordance with the concluded agreement for the required specialty.

    ^ The main results of the work of the KraSMA on the implementation of the tasks of the priority national project in the field of health care in 2006

    The implementation of the priority national project in the field of health care in the Krasnoyarsk Territory (hereinafter referred to as the Project), as in other constituent entities of the Russian Federation, is carried out with the following priorities: 1) development of primary health care; 2) development of the preventive direction of medical care; 3) additional medical examination of the population; 4) improvement of medical care for newborns and women during pregnancy and childbirth; 5) providing the population with high-tech medical care; 6) project management and information support.

    KrasSMA specialists are part of the headquarters for the implementation of a priority national project in the field of healthcare in the Krasnoyarsk Territory.

    In accordance with the decision of the expanded board of the Ministry of Health and Social Development of Russia dated 10/14/2005 "On the tasks of implementing a priority national project in the field of health care" at KrasSMA, like others medical universities, assigned the task of additional training, retraining and professional development of specialists.

    ^ Corporate events

    Within the framework of the project, the following organizational measures were carried out at KrasSMA: 1) amendments and additions were made to the training and production plan for advanced training and professional retraining of specialists for 2006; 2) mobile teams have been formed to train primary care physicians; 3) created working group on coordination of activities on staffing of primary health care.

    The plan of work of the Krasnoyarsk State Medical Academy for the implementation of the national project for the period up to 2008 was developed and approved.

    Agreements on cooperation in the field of training primary health care personnel were concluded between the Krasnoyarsk State Medical Academy and the Agency for Healthcare and Pharmaceutical Provision of the Administration of the Krasnoyarsk Territory; Health Administration of the Taimyr Autonomous Okrug; Health Department of the Evenk Autonomous Okrug.

    Taking into account the main directions of the implementation of the priority national project in the field of health care, the Action Plan for the development of postgraduate education for 2006-2008 was developed and approved.

    The academic laboratory "Family Health" has been created and is functioning (order of the State Educational Institution of Higher Professional Education KrasGMA Roszdrav dated March 17, 2006 No. 10-org).

    ^ Training (retraining), advanced training of general (family) practice doctors, district general practitioners and pediatricians

    For 2006, within the framework of the training and production plan for advanced training for 2006 (taking into account its changes and additions), as well as at the expense of the priority national project in the field of health, completed advanced training (professional retraining) 741 specialists primary health care(342 district therapists, 329 district pediatricians, 70 GP doctors) medical institutions of the Krasnoyarsk Territory, Taimyr and Evenk Autonomous Districts, the Republic of Tyva, the Republic of Khakassia.

    In the process of advanced training (professional retraining), the issues of organizing health care are covered by the teaching staff of the departments of public health and health care; health insurance, management and marketing; health economics and medical law; issues of HIV infection and immunoprophylaxis - in conjunction with specialists from KSUZ "Regional Center for the Fight against AIDS and Infectious Diseases".

    A definite innovation was the preparation of handouts for cadets, including legislative and regulatory documents related to the activities of a district therapist, district pediatrician, general practitioner, as well as regulations and legal acts on a priority national project in the field of health.

    Within the framework of advanced training cycles, it has become a tradition to meet therapists, pediatricians, obstetricians and gynecologists with the leadership of the health and drug supply agency of the Krasnoyarsk Territory Administration, the Krasnoyarsk Regional Compulsory Health Insurance Fund.

    In the process of field training courses, specialists from KrasSMA provide organizational, methodological and practical assistance to practical health care.

    Together with the Agency for Health and Pharmaceutical Provision of the Administration of the Territory, work was carried out to form a training base for the training of general practitioners in the Berezovsky District of the Krasnoyarsk Territory.

    This year, special attention was paid to the recruitment of specialists for postgraduate forms of education (internship, clinical residency). Admission to was carried out only on the basis of contracts for subsequent employment with a priority for primary health care and for high-tech types of medical care - training of specialists for the Center of Cardiovascular Surgery under construction in Krasnoyarsk (doctors - surgeons, cardiologists, transfusiologists, etc.).

    ^ Additional medical examination of the working population

    To ensure that additional medical examination of the population Krasnoyarsk Territory, working in state and municipal institutions of education, health care, social protection, culture, physical culture and sports and in research institutions in 2006, in GOU VPO KrasGMA Roszdrav in June this year, a cycle of thematic improvement "X-ray diagnostics of breast diseases" was organized and carried out (9 specialists were trained); in September-October - "Urology (for surgeons participating in clinical examination)" (31 specialists), in November - cycles of thematic improvement "Topical issues of endocrinology" (18 specialists), "X-ray diagnostics of breast diseases" (for specialists of the Republic of Khakassia).

    During the year, the scientific direction "Health of the Healthy" was formed with funding of up to 1.0 million rubles at the expense of the Krasnoyarsk State Medical Academy.

    As part of the implementation of this direction of the Project, additional medical examinations were organized and carried out for the employees of Krasnoyarsk State Medical Academy together with the Municipal Healthcare Institution "City Polyclinic No. 14"

    ^ Project management and information support

    Much attention was paid to information support of the Project... Since January 2006, monthly seminars, round tables with interns and clinical residents in the above-mentioned specialties have been held, where issues of implementing a priority national project in the field of health are discussed, including improving the work and strengthening of primary health care.

    In October of this year, seminars were held with 6th year students of pediatric and medical faculties, 5th year students of the Institute of Dentistry on the Project in the Krasnoyarsk Territory and upcoming employment after graduation educational institution... Prospective graduates are offered jobs in primary health care.

    Krasnoyarsk State Medical Academy implements the tasks of the Project in close cooperation with state and municipal executive authorities. So, on 02.09.2006, specialists of Krasnoyarsk State Medical University took part in a meeting of the association of heads of local self-government "Vostok", which took place in Kansk under the chairmanship of the Governor of the Krasnoyarsk Territory A.G. Khloponin. During the meeting, special attention was paid to the development of the "institute" of general practitioners in the region, the formation of an interdistrict medical center on the basis of the MUZ "Kansk Central City Hospital".

    Together with the Office of the Federal State Service for Employment of the Population in the Krasnoyarsk Territory, the Agency for Healthcare and Pharmaceutical Provision of the Administration of the Krasnoyarsk Territory, on September 26, a round table was held on the topic "Labor market in the healthcare sector in the implementation of the priority national project" Health ". The problems of staffing specialists with higher and secondary medical education of municipal health care institutions of the region, the goals and objectives of the employment service in the implementation of the national project "Health", the experience of KrasSMA in training primary health care personnel were discussed.

    As part of the professional development cycle "Public health and health care" at the MUZ "Berezovskaya CRH" (Krasnoyarsk Territory), a round table was also held on the topic "The National Project" Health "in the Krasnoyarsk Territory" with the participation of the Deputy Head of the Berezovsky District of the Krasnoyarsk Territory (T. A.), which discussed the organization of additional medical examination, immunization; provision of primary medical and obstetric-gynecological care to the population of the suburban area of the Krasnoyarsk Territory.

    In October this year, KrasSMA specialists took part in the work of field meetings (Lesosibirsk, Krasnoyarsk Territory) of deputies of the Legislative Assembly of the Krasnoyarsk Territory on the issue of "Development of health care in Lesosibirsk." During the meeting, it was decided to hold a presentation of graduates in Lesosibirsk for subsequent employment in December 2006.

    In November 2006, similar field meetings of deputies of the Legislative Assembly of the Krasnoyarsk Territory with the participation of specialists from Krasnoyarsk State Medical Academy were held in the Irbeysky and Sayansky districts of the Krasnoyarsk Territory.

    On October 26, 2006, with the participation of KrasSMA specialists, the implementation of a priority national project in the field of healthcare was discussed at the Coordination Council for postgraduate and postgraduate education of healthcare professionals in the Siberian Federal District, which took place in Tomsk.

    As part of the implementation of one of the main activities of Krasnoyarsk State Medical Academy: "Improving the system of advanced training (professional retraining) of specialists with medical education, focused on the regional characteristics of the Krasnoyarsk Territory" in October 2006, at a meeting of the Health Commission of the Legislative Assembly of the Krasnoyarsk Territory, the issue of additional funding was considered (at the expense of the constituent entity of the Russian Federation) advanced training (professional retraining) of specialists for 2007 in the amount of 10.0 million rubles.

    In general, it should be stated that all the points of the decision of the Academic Council of the State Educational Institution of Higher Professional Education KrasSMA Roszdrav dated December 21, 2005 on the issue “On the national priority project“ Health ”have been fulfilled.

    Thus, in 2006, the Krasnoyarsk State Medical Academy, within its competence, was actively implementing the tasks of the priority national project in the field of healthcare.

    ^ The main directions of improving postgraduate education for 2007

    In 2007, the implementation of the Action Plan for the Development of Postgraduate Education for 2006-2008 and the Action Plan of the Krasnoyarsk State Medical Academy for the Implementation of the Priority National Project for the Period until 2008 will be continued.

    The plan for advanced training and professional retraining of specialists for 2008 will be formed on the basis of the "Register of medical workers" database with a focus on the "budget - non-budget" technology, general improvement cycles in the amount of 144 hours and professional retraining cycles in the amount of more than 500 hours.

    Additional professional development of primary health care doctors (district therapists, district pediatricians, general practitioners) will continue, including under government contracts concluded as a result of an open competition for this work conducted by Roszdrav.

    In 2007, it is planned to improve the quality management system of postgraduate education.

    During 2007-2008, the technology of field training courses will be improved - not only training, but also comprehensive organizational and professional assistance to practical health care.

    The development of paid educational services.

    Thus, within the framework of improving the personnel training system, by 2009 it is planned to create a competitive system of postgraduate education on the basis of a unified education policy, taking into account the regional component, and the provision of medical care in the region.

    Thus, the main results of the work are as follows:

    1. Trained in internship 428 people

    2. Trained in clinical internship 127 people

    3. 4525 people underwent advanced training (professional retraining)

    4. An assessment of the employment of young specialists was carried out

    5. The KrasSMA website has a system for warning about vacancies in healthcare institutions of the city and region

    6. The tasks of the priority national project in the field of health have been fully implemented

    7. An action plan was developed for the development of postgraduate education until 2008.

    For the further development of a competitive system of postgraduate education on the basis of a unified education policy, taking into account the regional component, as well as the implementation of the tasks of the priority national project in the field of health in 2007, the following tasks are planned:

    1. Fulfillment of the state assignment for the training of primary care physicians

    2. Improving the quality management system of postgraduate education

    3. Improving the technology of conducting field cycles
