    for examination of children in PMPK

    Director: (stamp, signature)

    Director: (stamp, signature)

    Record on PMPK is carried out at submission of the documents necessary for PMPK. PMPK keeps a journal of children's records for examination. The parents (legal representatives) of the child are informed about the date, time, place and order of the survey, as well as about their rights and the rights of the child related to the survey, the commission is within 5 days from the date of submission of documents for the survey. Important: for registration at the PMPK, a compulsory referral from a kindergarten, a school, a polyclinic is not required, it is enough that the parents wish to go through the PMPK.

    To conduct a survey of a child, his parents (legal representatives) present a document proving their identity to the commission, documents confirming the authority to represent the interests of the child, and also submit the following documents (clause 15 of the Regulations on the Psychological, Medical and Pedagogical Commission approved by the Ministry of Education and science of the Russian Federation dated September 20, 2013 No. 1082):

    a) a statement of conduct or consent to the examination of the child in the commission;
      b) a copy of the passport or birth certificate of the child (provided with the presentation of the original or a copy certified in the prescribed manner);
      c) the direction of the educational organization, organization providing social services, medical organization, other organization (if available);
    d) conclusion (conclusions) of the psychological, medical and pedagogical consultation of the educational organization or specialist (specialists) carrying out psychological, medical and pedagogical support of students in the educational organization (for students of educational organizations) (if available);
      e) the conclusion (conclusions) of the commission on the results of the previous examination of the child (if any);
      e) a detailed extract from the history of the child’s development with the findings of doctors observing the child in the medical organization at the place of residence (registration);
      g) characteristics of the student, issued by the educational organization (for students of educational organizations);
      h) written work on the Russian (native) language, mathematics, the results of independent productive activities of the child.

    If necessary, the commission requests additional information about the child from the relevant authorities and organizations or from parents (legal representatives).

    If, in order to pass through the PMPK, you were asked (and you received) medical reports or pedagogical characteristics transmitted in sealed envelopes, you can safely open them. Parents, as legal representatives of a child, have the right to information about what conclusions are made regarding a child (this right is guaranteed by article 22 of the Federal Law “On the Principles of Health Protection of Citizens in the Russian Federation” and article 44 of the Federal Law “On Education in the Russian Federation”) . Another question is whether it is worthwhile to present at the PMPK only those conclusions that seem to us to be biased and describe only the complexities of the child, without revealing his abilities and capabilities. In such a situation, it is advisable to submit alternative conclusions, including from experts of non-governmental organizations. Do not believe the words that the conclusions of non-governmental organizations are not taken into account. In the event that it becomes necessary to appeal the PMPK’s opinion, it is important to provide information that there are different opinions of specialists and these opinions were presented to the PMPK.

    In Moscow, the “medical” part of the psychological-medical-pedagogical examination of PMPK is isolated from the process of psychological-medical-pedagogical examination of the PMPK. The child should come to PMPK with the conclusion of the special medical commission about whether he has limited health opportunities and with recommendations about the conditions for receiving education. The procedure for obtaining such a medical certificate in Moscow.

    Do not forget to attach to the package of documents the recommendations of specialists, which were obtained on the initiative of parents in the process.
